Targeted case management

Targeted Case Management connects individuals to the essential resources needed to live stable, healthier lives. Case managers help clients navigate systems such as housing, medical care, mental health services, education, employment programs, disability benefits, food assistance, and community supports. Services may include completing applications, attending appointments, crisis planning, advocacy, coordination with providers, and ongoing follow-up. TCM helps reduce barriers, increase independence, and ensure clients have consistent guidance while working toward long-term stability and self-sufficiency.

Psychosocial Rehabilitation

Psychosocial Rehabilitation is designed for individuals living with serious mental health conditions such as schizophrenia, bipolar disorder, major depression, or other persistent disorders. PSR focuses on rebuilding independence and improving daily functioning. Clients participate in structured skill-building sessions that may include life skills, social skills training, medication education, problem-solving, budgeting, hygiene, communication, and coping strategies. The goal is to help individuals increase stability, confidence, self-sufficiency, and community integration—allowing them to experience a higher quality of life.

Psychiatric Services

Psychiatric services are provided by licensed psychiatric professionals who evaluate, diagnose, and treat mental health conditions through medication, therapeutic support, and ongoing monitoring. During psychiatric evaluations, clients discuss symptoms, history, and goals to determine whether medication may be beneficial alongside therapy.
Medication management ensures that prescriptions are safe, effective, and tailored to each client’s needs. Our providers:
•Monitor symptom improvements and side effects
•Adjust medications as needed
•Coordinate care with therapists and primary physicians
Provide education on the medication purpose, dosage, and expectations
